Wound careWho this is for
Cannot get to a primary care office without real effort from them or from you. Uses oxygen, a walker, or a wheelchair. Has multiple chronic conditions being managed across several offices. Was recently discharged and needs someone watching the plan. Lives in assisted living or skilled nursing and needs a consistent clinical presence.
